Graduating Cadets

Graduating cadets may apply for a
Basic Firefighter Certificate of Achievement.

Instructions for filling out the application:

1. 1. Click and log into "My COD” Portal on the main website.
      
      
   2. Click "A&R Forms" located on the left side of the webpage.
      
      
   3. Select "Certificate Application."
      
      
   4. Please fill out the application completely.
      
      
   5. Click "Submit" at the bottom of the page.
